Illinois residents can officially begin filling out their medical marijuana applications.

The state of Illinois posted their nine-page medical marijuana applications online Monday. However, no applications will be accepted until Sept. 2.

The State has designated deadlines based on applicant’s last names. Patients applying with the last names of A-L must submit their medical marijuana application by Oct. 31. Patients whose last names are in the M-Z category have until Dec. 31 to apply, but applications will not be accepted until Nov. 1.

There are four separate forms for patient applicants: the patient’s application, a caregiver application (if applicable), a physician certification form, and a fingerprint consent form.

Applicants will also be required to pay an application fee. Patients must pay $100, unless they are enrolled in the federal Social Security Disability Income or Supplemental Security Income Disability programs. Then, the fee is just $50.

The State is still working on the application forms for dispensary and cultivation center applicants. According to the state of Illinois’ medical marijuana website, the forms are expected to be ready between Sept. 8 and Sept. 22.
